Blaenau Gwent

The team provides assessment and care management services that promote individuality and enables people with a learning disability to have normal patterns of life within the community and help develop their maximum potential. www.blaenau-gwent.gov.uk

Caerphilly

Daytime opportunity services provide support to people with learning disabilities to help them gain access to a wide range of community-based activities. These activities will promote and encourage choice, independence and inclusive engagement in local communities.  www.caerphilly.gov.uk

Carmarthenshire

Conwy

We provide an assessment and care management service for adults with learning disabilities whose disability prevents them from undertaking the usual activities of daily living without assistance or who are vulnerable to neglect, exploitation or abuse. www.conwy.gov.uk

Denbighshire

 The learning disabilities team provide a range of services including work opportunities and training, independent living, specialist health and education.

Newport

The vulnerable adults team is responsible for protection of vulnerable adults and operation of day care services and activities.  www.newport.gov.uk

Monmouthshire

Neath Port Talbot

Care and support for adults with a learning disability is provided by the Community Support Team based at the Old Civic Centre in Neath, this is a county wide team that provides the assessment and management of people within the borough.  www.neath-porttalbot.gov.uk

Pembrokeshire

Rhondda Cynon Taff

We work in partnership with other organisations to provide a range of services to help and support children and adults with learning disabilities, along with their parents and carers. We aim to help those who use our services to live a full and active life within the community, and to enable them to reach their full potential.

Swansea Social Services works in close partnership with other local agencies in Swansea in providing information and services to support people with a learning disability.
